01 Your details & order

Find the transaction ID on your PayPal receipt or in your PayPal activity. It lets us match your questionnaire to your payment.

02 Your business picture

All questions below are required. Estimates are welcome—label them clearly. If something doesn’t apply, say so.

Include how long you’ve been operating, your team size, and locations or service area.

List your main services, packages, memberships, and their prices.

Use the last three months if available. Include a breakdown of major costs such as payroll, rent, marketing, subscriptions, and debt payments. Label estimates and note whether owner pay is included. If you don’t know a number, say so.

Include your main marketing channels, monthly customer or appointment volume, repeat business, and whether you’re underbooked or at capacity.

Describe the issues you want this audit to address and what you think may be causing them.

Tell me what changed, what worked, and what didn’t. “Nothing yet” is fine.

Share your priorities for profit, owner pay, time, or growth—and any budget, staffing, or other limits the plan needs to respect.
